About the Role

Reporting into the Health and Wellbeing Manager, you will manage a portfolio of third party suppliers and chronic disease management programs to deliver according to the Health and Wellbeing strategy. You will ensure that HCF's Health and Wellbeing strategy is implemented to plan and is operationally efficient. 

Key Responsibilities include:

  • Manage and implement disease management programs for members, to achieve HCF’s objectives in line with the HCF, Member Health and Health and Wellbeing strategy
  • Manage and partner with key internal stakeholders to assist them with their health and wellbeing needs.
  • Monitor and report on effectiveness of the disease management programs
  • Support Frontline staff with member enquiries regarding current disease management programs
  • Recommend enhancements/expansion/rationalization to current programs to ensure uptake, effective disease management and risk identification

About You

  • Strong organisation and relationship management skills
  • Excellent Project management knowledge and ability to use excel proficiently
  • Excellent verbal and written communication skills
  • Ability to streamline and manage processes 
  • Clear focus on the market in regard to member’s needs and expectations
  • Experience with health & wellbeing programs would be an advantage
